This form should be filled out when you are requesting an ordained minister of Life Changers Church to officiate a funeral or memorial service.
The minister will meet with your family a 2-3 days prior to the service to receive key facts and information you would like shared about your loved one to use during the service.
For the service, the minister will do their best to help prepare your immediate family for the service for your loved one. He/she reserves the right not to officiate the service.
