BEFORE SUBMITTING THIS FORM, PLEASE VERIFY THE BUILDING IS AVAILABLE BY REVIEWING OUR RESERVATION CALENDAR HERE.

By submitting this form, you agree to the terms and conditions of using space at Flagstone listed here:

1) Must submit request at least 2 weeks prior to event.
2) Attendees will stay only in the rooms of the building in which you have reserved and stay out of the worship center and offices.
3) No minor children will be left unsupervised.
4) We will leave the building as we found it, and complete the cleaning check-list provided to me. 

Usage Fees:
Members - none
Non-Members - $75
