The undersigned parent/legal guardian herby grants permission to Faith Community Bible Church for my youth(s) listed below to take part in any Youth Group Activity.
Should my youth require immediate or emergency medical care while engaged in an activity sponsored by FCBC, in my absence, I hereby grant FCBC authority to release my youth for medical treatment to such medical personnel as FCBC determines appropriate under the circumstances.
In consideration for the privileges of allowing my child to participate in youth group activities, I agree to release and hold harmless FCBC and its officers and agents, from any liability to or responsibility for bodily injury, damage or illness to the below identified youths while participating in any youth athletic or social activity which may be directly or indirectly sponsored by the church. Further, I agree to indemnify and hold harmless FCBC, its officers and agents with respect to any claim asserted by or on behalf of my youth as a result of bodily injury, illness, or damage.
I give permission for photos or videos of my youth to be used by FCBC for promotional purposes. I understand that my youth's name will not be published with images.
